Grant Avenue Capital-backed Fortis Health completed the acquisition of Physiocare Hospice.

Grant Avenue Capital-backed Fortis Health, a company offering home health, hospice, palliative and personal care services, completed the acquisition of Physiocare Hospice, an Indiana-based company offering home health care services. Financial terms were not disclosed. “Physiocare is a strong company with an almost 15-year history of serving its community.
