Revelstoke Capital-backed ClareMedica Health Partners completed the acquisition of Plaza Medical Centers.

Revelstoke Capital-backed ClareMedica Health Partners, a provider of diabetes care, cancer screening, blood pressure management and mental health services for senior citizens, completed the acquisition of Plaza Medical Centers, a physician-led and managed organization that focuses on providing preventative and comprehensive care to patients. Financial terms were not disclosed. "ClareMedica is excited to partner with the team at Plaza.
